Youth housing and reintegration service and after care

The Youth Housing and Reintegration Service including After Care Service (YHARS) provides brokerage services and a coordinated and integrated case management response to the complex needs of young people aged 12 to 21 years who are homeless or at-risk of homelessness.

The guide outlines service provider roles and responsibilities, principles for information sharing and relationship management with government and non-government agencies involved in the delivery of services to YHARS clients.

The guidelines also outline referral processes and criteria, principles for case management, accommodation support and working with young people, the administration of brokerage funds, record keeping and conflict resolution.

YHARS help for young people

Youth Housing and Reintegration Services (YHARS) help young people who are homeless or at risk of becoming homeless. They provide support, financial help, and access to different accommodation options.
