The Systems and Practice Review Committee (SPRC), is a departmental committee which oversees all Systems and Practice Reviews. The SPRC is responsible for approving and making findings and recommendations in Systems and Practice Review Reports.
Following consideration of the review by the SPRC, copies of the final report are emailed to all departmental participants and relevant managers and directors, to allow critical discussion and reflection on learnings. In some cases the reports may be emailed to relevant managers who are then responsible for disseminating the findings of the review to staff.
A de-identified copy of the report may also be disseminated to a wider audience if the SPRC deems the learnings relevant.
All reviews, together with all material used in the preparation of the report, are provided to the Child Death Case Review Panel (CDCRP), which is responsible for the oversight of the review process undertaken by the department. The CDCRP is drawn from a pool of approved panel members with expertise in a diverse range of fields related to child protection. The CDCRP has members from the department, other government departments and non-public service professionals. The CDCRP reviews the departmental report and may make further recommendations.
Where the death of a child is defined as a reportable death under the Coroners Act 2003, a de-identified copy of the review report is provided to the State Coroner. ‘Reportable deaths’ include deaths in unexpected circumstances and all deaths of children in out-of-home care. The Coroner reviews the cause and circumstances of the child’s death, and this may include consideration of information about the department’s involvement.
